Associations can apply to become an approved Medavie Blue Cross Association

In order to facilitate the assessment of your association for approval, this application must be completed, signed, and include all required documentation to support your answers. Please indicate in your answers where the supporting documentation can be found in your overall response (i.e. appendix #, document name, page, paragraph, etc.) The application is also available in a modifiable PDF format upon email request to provider@medavie.bluecross.ca.
